Which fetal positions are considered most favorable for vaginal delivery?

ROA and LOA

Fetal head orientation in the birth canal and how it aligns with the mother’s pelvis determines how easily labor progresses. The most favorable positions are when the occiput is facing the front of the mother (occiput anterior). On the left or right side, this becomes left occiput anterior or right occiput anterior. In these LOA or ROA positions, the head can flex efficiently to present the smallest diameter (the suboccipitobregmatic diameter), and as it descends, it rotates smoothly to align with the pelvic curves. This reduces resistance, shortens labor, and lowers the need for interventions.

Positions where the occiput is toward the sides with a transverse orientation (occiput transverse) are less ideal initially. They can lead to longer labor and more back pressure or difficulty with rotation, though many babies can still rotate to an anterior position during labor. That's why LOA and ROA are considered the best for vaginal delivery.
